Set symmetrical sculpt to an axis (this uses X). To create a nose, beak, or other single protuberance, use the Grab, Draw or Inflate tool on the CENTER of symmetry. Using the cursor on either side can start horns, arms or other symmetrical protuberance (one on either side), but if that's not the desired result, it's pretty annoying. Happy Sculpting!
